1/19/2023 EnchantaVaughn Approved Behavior: As the client prepared for family session, he was seen helping his mom put away groceries. Client greeted QMHP with welcoming gestures saying good afternoon. Client mood was viewed as vibrant, upbeat, and energetic. Client displayed optimism going into his session. Client was observed being a good listener while QMHP reviewed his objectives and goals. Client was monitored and supported for exhibiting the correct manners and social skills. Client displayed appropriate body cues and voice volume in social setting. Client displays good social behaviors, laughing and playing safely in the home. Client was doing an amazing job coping and staying calm during challenging tasks. Client affect was sunny, as characterized by his smile and happy facial expressions. Client shows excitement with talking to his counselor about his day. Behavior: Client expressed happy expressions and was respectful to the mental health professional assigned to his case. Client was able to role play skills he learned in yesterday’s psycho social and implement them in today’s psycho social. Client was praised for his hard work and commitment with learning and retaining information from the session. Client have been trying more and showing more effort in the home. Client is very nice to his mother and friendly to his siblings. Client was observed displaying positive social behaviors in the home. Client was in a fantastic mood, as evidenced by his energetic and fulfilled spirit. The client was supported for displaying an understanding of the social skills that were reviewed. Client verbalized positive I statements by saying “I feel good” and “I will have a good day”. Intervention: QMHP will conduct evening session to debrief and talk about a plan to follow and ways to reduce tension and negative energy so that he can have a successful and positive day. QMHP provided support and encouragement with client staying on task in the home. QMHP applauded client for being a leader. QMHP reiterated to client the benefits of following the rules and of the privileges he can receive when he does so. QMHP assisted client with finding and utilizing necessary resources in his personal environment. QMHP provided simple non-verbal praises as client was observed displaying positive behaviors. QMHP prepared client for therapeutic activity. QMHP applauded client for his commitment and efforts with doing his best and having a great time in his session. Intervention: QMHP motivated client to respond to exercise and express his feelings appropriately. QMHP offered active listening skills as needed. QMHP provided client with encouragement to demonstrate the positive behaviors and expectations they had been talking about. QMHP encouraged client to communicate to his father when he needs help. QMHP praised client for staying on task. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of the assignment. QMHP supported client displaying positive social interaction with peers and authority. QMHP will brainstorm strategies with client and encourage self-monitoring. QMHP will provide mediation and relaxation techniques, as well as, simple praises to help build high self-esteem and challenge client to do good on his own. Response: Client was open and transparent with his counselor. Client promised to follow his safety plan and reduce unnecessary tension/drama in the home. Client exhibited a positive attitude toward mother and counselor. Client smiled and hugged QMHP for being there for him when he needed. Client displayed positive interaction with his siblings and was supported with building healthy relationships with his family. Client exhibited good manners and was polite throughout the session. Client implemented the positive reinforcement provided by his mother and was able to stay motivated and enjoy his session. Client utilized the resources provided and seem to be doing better with managing symptoms of ADHD. Client has shown he was capable of having a successful day today. Response: Client transitions smoothly to the next activity, displaying appropriate behaviors. Client expresses good statements about his day saying he was doing his work, using his manners, and being fair to others. Client is more confident, as evidenced by assertive and proud tone. Client have been doing a better job controlling impulsive and hyperactive behaviors, due to keeping his hands to himself and complying to home rules. Client reduces sadness and finds more excitement in learning about different ways to control his reaction to certain situations and be more receptive and open in therapeutic setting. Client was mindful of his family needs and made sure to thank them for giving him more opportunities to take care of himself mentally and emotionally.
